Please send the following information to MDE-OST@Michigan.gov to complete the Intent to Apply.
The legal name of the organization.
The MEIS ID for the person who will be completing the application (see here to create an account or find your previous ID https://mdoe.state.mi.us/meis/login.aspx).
The email of the person whose MEIS ID was provided above.
The number of sites the organization will be applying for.
Regardless of when the Intent to Apply is submitted, the application is due by 12 noon on October 6, 2022. No late applications will be accepted. It may take 24 business hours from the Intent to Apply to receive access to the application.
